La Carreta #6 records the week's highest violation total with 17, while three establishments complete routine inspections without a violation.
The Central Virginia Health District conducted 22 routine food inspections across Greater Lynchburg during the Aug. 4–10 reporting period, with inspectors recording 82 total violations.
La Carreta #6 in Forest recorded the week's highest violation total with 17, followed by Papa Gallo in Lynchburg and Dale's Diner in Montvale with eight each, Joe Bean's in Bedford with seven, and Texas Roadhouse in Lynchburg and Country Cooking By Irene in Lynchburg with five each.
Three establishments completed routine inspections without a violation: Hado's Authentic Lebanese Kitchen in Moneta and two University of Lynchburg vending operations.

Highest Routine Inspection Violation Totals
La Carreta #6 — Forest (17 violations)
La Carreta #6 recorded the week's highest violation total during its routine inspection.
Inspectors cited problems involving managerial oversight, handwashing facilities, cross-contamination, cooling and cold-holding temperatures, date marking, chemical storage, cooling methods, wiping-cloth and utensil storage, dishware handling, sanitizer test strips and lighting.
Several food-safety issues involved temperature control. Containers of queso measured between 46°F and 48°F, while refried beans prepared the previous day measured between 51°F and 56°F. Birria, spinach and chipotle sauces were also above required temperatures. Inspectors found additional cold-held foods above 41°F, including black beans, cooked pork, sour cream and enchilada sauce. A container of queso left on a cook-line shelf measured 63°F. Foods affected by improper cooling or cold holding were discarded.
Inspectors also observed employees using the same utensils while working with raw and ready-to-eat foods and found raw eggs, pork and chicken improperly stored above other foods. The foods were rearranged and employees began using separate utensils. Other corrections made during the inspection included removing cans of household insect spray, removing improperly stored utensils and rerunning glassware through the dish machine.
Papa Gallo — Lynchburg (8 violations)
Papa Gallo received eight violations during its routine inspection.
Inspectors cited an open employee drink above a prep area, improper use of hand sinks, inadequate hot water at one hand sink, raw eggs stored above cooked refried beans, cleanliness issues involving a freezer and ice machine, improper cold holding, inadequate procedures for horchata held outside refrigeration and clean serving bowls stored where they could be contaminated.
Temperature control was among the more significant findings. Raw shrimp measured 50°F, while raw chicken and raw beef in a grill drawer measured 56°F. Management called for maintenance, moved affected foods to other refrigeration and stopped using the drawers for TCS foods until they could maintain proper temperatures. Several other violations were corrected during the inspection.
Dale's Diner — Montvale (8 violations)
Dale's Diner received eight violations during its routine inspection.
Inspectors found the restaurant did not have an active certified food protection manager, raw meat and eggs were stored above prepared spaghetti, and sanitizer in the three-compartment sink measured 100 ppm, below the required concentration.
Other findings included an unscreened open window, a deteriorating refrigerator gasket, sanitizer test strips that had expired years earlier, apparent mold or mildew on the walk-in refrigerator ceiling, grease and dust buildup and exposed studs and electrical components in the mop closet.
Several issues were corrected during the inspection, including relocating the spaghetti, increasing the sanitizer concentration to the proper level and closing the unscreened window.
Joe Bean's – Bedford — Bedford (7 violations)
Joe Bean's received seven violations during its routine inspection.
Inspectors reported a significant number of flies in the establishment, including flies landing on lids, in-use scoops and other surfaces. The report noted that drive-thru windows were being kept open during orders and an employee left a back door open.
Other violations involved the employee health policy, uncovered food items, improper handling of a sanitized spoon, deteriorated and rusting material beneath the three-compartment sink, a plumbing leak and an overflowing dumpster.
Several immediate corrections were made, including removing exposed scoops for rewashing and sanitizing, covering food items and resanitizing the spoon.
Texas Roadhouse — Lynchburg (5 violations)
Texas Roadhouse received five violations during its routine inspection.
Inspectors found unlabeled bulk food-storage containers, no beard restraints for line-prep food workers, stained and deeply cut cutting boards, grease buildup in exhaust hood filters and convection ovens, and broken floor tiles or missing grout throughout portions of the kitchen and dishmachine area.
The food-storage containers were labeled during the inspection. Management was instructed to address the remaining equipment, cleaning and facility-maintenance issues.
Country Cooking By Irene — Lynchburg (5 violations)
Country Cooking By Irene received five violations during its routine inspection.
Several flats of eggs were being stored at room temperature and measured 77°F. Inspectors also found the reach-in refrigerator and foods inside above required cold-holding temperatures. Soup measured 59°F and gravy measured 53°F.
The inspector directed the establishment to discard TCS foods from the reach-in and said it should not operate without adequate refrigeration capable of supporting its menu. The eggs were moved to refrigeration.
Other violations involved employee-health documentation, potatoes stored on the floor and single-service plastic knives stored with the food-contact ends exposed to handling.
